Use this gift certificate form to purchase a gift certificate for someone. These gift certificates can be redeemed at my office location only, but if an outcall “Nonsmoking homes” only is needed to a client’s home, then there will be an extra fee at time of scheduled massage or a special invoice can be sent to purchaser to accommodate that situation. Gift certificates are also available at my office: 2600 U.S. 1 South St Augustine, Fl 32086 by setting up a time to purchase between clients. Paypal is available on website as well as the option of using your own credit card or you can purchase by phone. They are mailed out the next day & you will receive an email confirmation. Gift certificates are not refundable, but they are transferable to another person. There is a 6 month expiration date which started with any certificates purchased 6.1.15 or after.
Any previously purchased certificates will of course be honored, but they need to be redeemed at the office only. The recipient of the certificate needs to mention it upfront when booking the appointment & present it at the office before the massage starts so that the correct massage will be given. They can only be redeemed by Diana, the owner, not thru any other therapist, so they are not available to couples’ sessions unless permission given by the oHalfwner & an appointment scheduled in advance. It is my right as a business owner to dishonor any certificate if there is rude, offensive, or disrespectful behavior when booking or redeeming. SOOTHING TOUCH SPORTS OR AROMATHERAPY MASSAGE OILS
These water dispersible massage oils smell great, as well as being paraben-free. The aromatherapy lite oil is a blend of essential oils of lavender, ginger, geranium, rosemary, thyme, spruce, and ylang ylang in carrier oils of vitamin E, sunflower seed, grapeseed, sweet almond, and rice bran. The Sports lite oil as a blend of essential oils of geranium, orange, peppermint, eucalyptus, clove, fennel, and white camphor in carrier oils of vitamin E, sunflower seed, grapeseed, almond, and rice bran with arnica added. Both oils are parabens free and vegan. They are used during all massages unless specifically requested for nut and fragrance free.
